Secondary Metastasis to L5

60-year-old lady, came to us with chronic lower backache for few months. She has history of fall on sitting 2 months ago. She also complaint of difficulty in passing motion for weeks, and she had to rely on laxatives regularly.

On examination, she had local tenderness over L4/L5. Lumbosacral X-ray was ordered.

Lateral X-ray showed abnormal L5. AP view showed loaded faecal matter.

She was then referred to tertiary center for MRI.

Final Diagnosis: Secondary bone metastasis from breast carcinoma.
