Ruptured Left Tympanic Membrane with Hearing Impairment

48 years old gentleman, came to us for annual audiometry. Audiometry results as below:

Audiogram done showed bilateral hearing impairment with average frequency 0.5, 1.0, 2.0, 3.0 >25.0.

Right side – likely noise-induced hearing loss with dip over high frequency at 6k and 8k.

Left side – showed dip over all frequency range, which is not common in noise-induced hearing loss unless in the late stage, and noise-induced hearing loss likely to be bilateral too.

Right – Left

Thus, we carried otoscopy and noted this gentleman to have ruptured left tympanic membrane(see pic below).

Right otoscopy – unremarkable.
