46 years old lady, came to us with symptoms of anaemia – giddiness, lethargic and shortness of breath. She has been having prolonged heavy menses for the past 2 weeks.
On examination, this lady appeared pale. Her vitals were still stable with:
Blood pressure : 90/60mmHg
Heart Rate: 90/min, strong volume
Full Blood Count was done stat(please see below):
Haemoglobin was at 47g/L(or 4.7g/dL), with low MCV/MCHC.
Per abdomen, suprapubic mass was felt at about 16 weeks size.
Bedside ultrasound showed huge uterine fibroid(see pic below):
Uterine fibroid measuring 12.3cm x 10.4cm
In view of ongoing blood loss through her menorrhagia, we have arranged emergency referral for her to see gynaecologist in the tertiary center.
Final Diagnosis: Severe Anaemia secondary to menorrhagia due to huge uterine fibroid
